Deduction codes

Deduction codes can be found for both active and retired Detroit Police Officers. You will need your most recent pay stub.

Life Insurances:

5DETROIT POLICE BENEFIT AND PROTECTIVE ASSOCIATION  -  (313) 870-9301

A.  Active code: 40620  $8.56 bi-weekly               Retired code: 40315 $18.55 monthly
B.  $15,000 Death benefit less any return of assessments paid to member.
C.  $7500 Additional death benefit if the death is determined "in the line of duty' by the Department.
D.  $500 - $4000 Paid to member upon a duty or non-duty disability retirement.
E.  All assessments paid to the member on their 25th year of membership less any disability return of assessments.
F.  Members cease paying assessments on the last day of the month in which they reach their 40th year as a member.

B.  $5,000 Death benefit for dependent spouse and children 6 months - 19 years of age.
C.  $500 Death benefit for dependent children 14 days - 6 months of age.
D.  Policy value can be increased during open enrollment at Police Personnel.
E.  Plan may be continued after retirement within 30 days of separation.

C. $10,000 Death benefit for spouse whom your legally married to and children from live birth to 26 years of age.
D. Plan may be continued after retirement within 30-days of separation.
E. At age 65, premium and coverage cuts in half - no change in dependent coverage.  YOU MUST notify the DPOA office at least
    30 days  prior to your 65th birthday.

DETROIT POLICE OFFICERS DEATH BENEFIT PLAN

Active DPOA Members
Code 40630  $2 Deducted from paycheck when an active member belonging to the plan dies on/off duty. The total amount of death benefit depends on the number of participating members at time of death.

Code 40735 -  $20 Mandatory participation. The benefit amount of the policy depends on the number of DPOA members. Deducted only when an active DPOA member dies on or off duty.

Active LSA Members
Code 40730 -  $10 Mandatory participation. The benefit amount of the policy depends on the number of LSA members. Deducted only when an active LSA member dies on or off duty.

D. $500 Death benefit for dependent children 15 days - 6 months of age.
E. At age 65, premium and coverage are reduced - no change in dependent coverage.
F. Plan may be continued after retirement within 30 days of separation.
