Do you need to change your beneficiary?
Beneficiary Certificate now available online
For your convenience, Member's of Detroit Police Benefit and Protective Association can now download and print a change of Beneficiary Certificate. Once you complete the form and have it notarized, you must mail it back to the Association.

WHEN NAMING YOUR CHILDREN:
When naming your children as your beneficiary, the Member's name must appear on the child's birth certificate. If the Member's name is not on the birth certificate, the child will have to obtain a DNA test to determine the Member-child probability of parentage from a properly accredited institution such as American Association of Blood Banks or American Society for Histocompatibility and Immunogenetics.
Before the member's death, the Member can complete an Affidavit of Parentage to have his name added to the child's birth certificate or the Member can complete an Acknowledgement of Parentage form. Contact the State website in which the child was born to obtain the Affidavit of Parentage or the Acknowledgement of Parentage.
WHEN NAMING YOUR TRUST:
In the event that a Member wants to name their Trust as beneficiary, the Association recommends that the Member obtain his/her own counsel regarding the propriety of designating the Trust as a beneficiary. In this connection, the Board of Trustees has the authority under Article VII, Section E of the Articles of Association to offset against any death benefits payable on behalf of any Member, the Association's reasonable attorney fees and litigation expenses which the Association incurs in the resolution of any dispute which arises because of an improper beneficiary designation, or which arises over the issue of who is a proper beneficiary of such death benefits.
It is the practice of the Association to send the complete copy of the deceased Member's Trust and most recent Beneficiary Certificate naming the Trust to the Association's legal counsel for interpretation of the Trust's compliance to Article VI. Again, all legal fees are deducted from the Member's death benefit.
Please note, the Association Bylaws prohibit any death benefit payment to step-children, ex-spouses, and significant others.
